There were rare cases in Face Offs where a question would get thrown out for one of two reasons. One the contestants couldn't think of an answer after 4 tries or two, someone gave away an answer (this only happened once).

Russia[]
The rule of throwing out questions is not applied in Russia. However, the Triple Game of the episode from April 24, 2010 was a unique case where neither of the teams scored any points. The question was: "How did you understand that the guests at the table ate too much?" Nine strikes occured at the buzzer, and the host even opened some answers before an answer was finally given. That was followed by more strikes and more answer openings, until the decision was made to not give the 213 points at stake to anyone. Link
